Before You Visit
I am currently in the office Mondays - Fridays.
New Patient Appointments are available at the following times: 1:00, 2:00, 3:30, and 4:00. Additionally, on Monday, Tuesday, Wednesday and Friday evenings there is a 5:00 appointment available. At this time this is the latest appointment I have available for new patients. If you need something later, please contact me and I will see what I can do.
Be sure to let me know if this will be a first appointment for you, as that usually requires I schedule a 1 hour block for you. Follow-up appointments are about 30 minutes long.
Thank you for your help with this process. Due to the high number of requests I receive, I am not always able to return these calls immediately. If I hear from you by email that you understand the fees and which insurance company you wish to use, it will help things move along faster.

All Visits | Weekdays Mon-FriInitial visits are 1:00 hour, and return visits are approximately 30 minutes in length.

This information will help me see you as the whole person that you are, rather than just a set of symptoms with a diagnosis.
You will have a chance to ask me any questions you have. If you like, I can give you my impression of what is going on and some ideas of treatment options going forward. From this first visit you will see how you and I relate to one another and if you'd like us to set out together on this journey towards better health and well-being. Hopefully along with letting me get to know you better, you might gain some insights and learn something new about yourself. This first visit can feel like a lot, and I want to make your experience as rich as possible. Towards this, I may ask you to complete some questions before our visit. I will make these available to you either through the mail or electronically. If at any time you have questions or concerns, please feel free to discuss them with me. |
Fees
Payment is due in full at the beginning of each visit. We accept payment in the form of credit/debit card, and check.
Billing Insurance: We can bill your insurance as a courtesy, in addition to, but not instead of, payment at the time of service. You may need to pay for your appointment in full until your deductible is met. It is your responsibility to find out if you have a deductible and whether it has been met before your appointment. Please check your specific plan for information such as:
We are currently in-network with Aetna, Ambetter, Amerigroup, Blue Cross Blue Shield of MS, CAQH, Cigna, Humana BH, Magnolia Health (Cenpatico), Medicaid, Medicare, Multiplan- PHCS, Three Rivers, Tricare Prime, Tricare Standard, United Health Care (Opum) & Wellcare. |
All other insurances:
If we are an out-of-network provider, many insurances do not cover our entire fee, and the remainder will be your responsibility. Until we have submitted claims to your insurance, we do not know how much your insurance will pay. For this reason, we ask that you pay in full at the time of your visit, and if there is an insurance over-payment, we can credit you this amount after the claim has been processed. Please let me know if this will be a hardship for you. Appointment fees may vary based on longer length of visit, greater than 30 min for a return visit, and if it is a visit of higher complexity, with more follow-up needed outside of appointments.
